Applying for Admission to MPH Program

the top of this page. Submit
the application and supplemental
information requested to MPH Program (address is on form).
- Download the the Rating Form found in the links at the top
of this page and request three individuals to complete and submit
to MPH Program (address is on form).
- Application to UT Graduate Studies, application fee, and official
transcripts must be submitted
directly to Office of Graduate Admissions. You may apply
online at http://admissions.utk.edu/graduate/.
Please observe application deadlines:
- February
1st for summer admission
- April 1st
for fall admission
- October
1st for spring admission
